Assists in the processing of applications for new or renewal of the U.S. Passport Book or Passport Card required for all travel outside the U.S.

Overview

The Passport Book is an internationally recognized travel document that verifies the identity and nationality of the bearer. A valid U.S. Passport Book is required to enter and leave most foreign countries.

You have the option to apply for a passport book, a passport card, or both. The passport card may ONLY be used for land and sea travel between the United States and Canada, Mexico, Bermuda and the Caribbean.

Passport applicants must be in the Clerk's Office prior to 3 p.m. to complete paperwork.

The U.S. Passport Book

To apply for a new U.S. Passport Book you may come to Room C119 in our DeLand Office. All applicants must appear in person if you do not meet the requirements for renewing by mail. Thoroughly reading the information on this page will inform you of all items you need to bring to the Passport Department. Passport applications are also accepted in several area post offices. You can download the application online from the U.S. Department of State.

Special requirements exist for children under the age of 16. Please call our office for assistance.

You must be an American citizen and submit proof of citizenship (U.S. Birth Certificate bearing the registrar's signature and seal or Naturalization Certificate). Effective October 30, 2010 the U.S. Department of State will only accept Puerto Rico birth certificates issued on or after July 1, 2010 as primary evidence of U.S. citizenship. You also need to submit the front and back of the proof of identity (Driver's License, military card) and one color passport style photograph taken within six months. Passport applications must be legible and completed in black ink.

Adult Passports Book are valid for ten years from the date of issue for adults and five years from the date of issue for minors. Note: Passport Services require personal appearance of all minors applying for U.S. Passports along with the minors parents or parent.

Application for a new U.S. Passport Book

You must be an American citizen and present proof of U.S. citizenship at the time of application. Proof of citizenship consists of a previous U.S. passport or a certified copy of your birth certificate, bearing the registrar's signature, date of certification, the name of parents or parent, and a raised, embossed, impressed or multicolor seal. If you are a naturalized citizen or born abroad, you must present a Certificate of Naturalization, Certificate of Citizenship, Report of Birth Abroad of a U.S. Citizen, or a Certificate of Birth (Form FS-545 or DS-1350).

One (1) passport style photograph taken within six months must be submitted with the application. Only a color photo will be accepted and must be 2" x 2" in overall size. The photograph must provide a clear likeness of the applicant and a frontal view without dark glasses or a hat. The background should be white or off-white. Photo service is available at Room C119 in our DeLand Office for a charge of $10.65. Photos made by an automatic booth type camera are not acceptable.

U.S. Passport Book Renewal

To be eligible for a renewal by mail, you must meet the following criteria:

  • Your most recent passport must have been issued to you no more than 15 years ago and you must submit that passport with your application.
  • You were at least 16 years of age when your most recent passport was issued.
  • Your name has not changed since your most recent passport was issued except by marriage or court order. You must submit documentation evidencing such a name change.

The Clerk's office does not execute renewal applications, you must apply by mail. Complete instructions are located on the application.

The U.S. Passport Card

The U.S. Passport Card is a wallet-sized card that is valid for international travel by land or sea between the United States, Canada, Mexico, the Caribbean and Bermuda only. It is not valid for travel by air to or from any foreign destination.

The U.S. Passport Card is subject to the same requirements as the U.S. Passport Book. Applicants must provide documents that attest to U.S. Citizenship such as birth and naturalization certificates.

You may apply for both the U.S. Passport Book and U.S. Passport Card at the same time. The U.S. Passport Card may be used to apply for the U.S. Passport Book.

Passport Card application forms are available in our Passport Department located in Room C119 of our DeLand Office, or any Passport Application Acceptance Facility. Forms for the passport card are online from the U.S. Department of State..

First time adult and minor applicants under the age of 16 must apply in person at a Passport Application Acceptance facility and will need to submit a completed Form DS-11 "Application for a U.S. Passport", one (1) photo, evidence of citizenship, proof of identity, (front and back of driver's license, military card), the application fee for the Passport Card and the execution fee. Passport applications must be legible and completed in black ink.

Adult applicants with a fully valid U.S. Passport Book can apply for the Passport Card by mail by submitting Form DS-82 "Application for a U.S. Passport By Mail", one (1) photo, a fully valid passport issued within the last fifteen years, and the passport application fee at a cost of $30.

U.S. Passport Cards are valid for 10 years from the date of issue for adults and five years from the date of issue for minors.

The processing and issuance of the U.S. Passport Card may vary. It may take as long as 8 weeks for the Passport Services to process applications. Neither overnight return delivery service nor Priority Mail service will be available for the Passport Card. Passport cards will be mailed to applicants using First Class Mail service.

All customers traveling within 14 Calendar days may make an appointment at a Regional Passport Agency by using an automated telephone system. The new automated appointment system permits customers to schedule appointments from their homes or any other locations 24 hours a day, 7 days a week, up to 3 days before their departure. No Fee Passports

No fee passports will be issued to eligible customers (i.e., officers or employees of the United States traveling on official business, etc.) in book form only.

Obtaining a Certified Birth Certificate
